New Health Form Requirements from BSA
Please complete the Annual Health Form below. This is to be completed for ALL registered youths and registered adults.
Annual BSA Health Form parts A, B and C
NOTE:
Cub Scouts only require Parts A and C unless you send your son to resident camp.
Parts A and C do not require a doctor’s signature. Simply complete the necessary information and have a parent or guardian sign the form.
Part B must be signed by a physician.
Please return completed parts A and C to your Den Leader as soon as possible.
These need to be in place according to BSA rules by January 2010.
FOR THOSE ATTENDING RESIDENT CAMP:
Part D on Page four of the site below is an additional section that will be needed if your son participates in a RESIDENT camp within Connecticut.
